In addition to being available for Kyushu University students and staff, the online appointment booking system can also be used by related individuals (such as parents or staff members) who need to consult about issues concerning students or staff. The content filled in the form will be reviewed by staff at the Center for Health Sciences and Counseling (including doctors, counselors, mental health social workers, disability support staff, public health nurses, etc.), and you will receive a reply via email or phone within approximately one week after submitting the appointment request (response times may be delayed during holidays). If you do not receive a reply within a week, please contact us using the information below.
线上预约系统除了供九州大学的学生和教职员工使用之外，其他相关人员（家长或教职员工）需要就学生或教职员工的问题进行咨询的时候也可以使用本系统进行预约。所填写的内容将由校园生活与健康支援中心的工作人员（医生、心理咨询师、精神健康社工、残疾人支援工作人员、保健护士等）确认后，在预约申请提交之后的大约一周内通过邮件或电话进行回复（如果遇到假日，回复时间可能延长）。如果在一周内未收到任何回复，请使用下面的查询方式与我们取得联系。

Specific Process after Filling out the Online Appointment booking System
- Fee:
All consultations are free of charge. - Confidentiality Agreement:
The contents of the consultation will be kept strictly confidential. However, the staff of the Center for Health Sciences and Counseling work as a team, and information may be shared among staff when necessary. Additionally, to improve the effectiveness of support, with the consent of the consulter, information may sometimes be shared with related departments. To enhance management and service quality, anonymized information may be used as foundational data for business improvement or internal meetings. For the purpose of providing better support, consultation details, with personal information masked, may be shared with other experts for feedback. In rare exceptions, if the consultation reveals a risk of self-harm or harm to others, or a high risk of abuse or violence, we may contact family members, doctors at the Center for Health Sciences and Counseling, academic supervisors, hospitals, or the police. In such cases, we will make every effort to obtain the consulter’s consent in advance. - Consultation Records:
The Center for Health Sciences and counseling will keep a record of the consultation, including personal information such as contact details, in a consultation records database. This information is used to help the center better understand the situation of the consulter and provide appropriate support. The consultation record will not be used for grading or any other purposes. Additionally, no one outside of the Center for Health Sciences and counseling staff will have access to the consultation records. The records will be stored for up to 10 years after the last consultation. - Research Activities:
To improve the consultation process and the effectiveness of support, the Center for Health Sciences and counseling conducts various research activities. Anonymized information may be used to present research findings at academic conferences or in scholarly journals. When such information is used in individual research projects, notifications will be posted on the Center for Health Sciences and counseling's website, so please check the site for updates.（https://www.chc.kyushu-u.ac.jp/)

校园生活与健康支援中心会记录咨询内容，包括联系方式等个人资料会被一起保存在咨询记录数据库当中。这些信息是用于帮助校园生活与健康支援中心更好地理解咨询者的情况并提供适当的援助。咨询记录不会用于成绩评价或其他目的。此外，除校园生活与健康支援中心的工作人员外，其他人无法访问咨询记录。咨询记录的保存期限为最后一次咨询后的10年之内。 - 研究活动
校园生活与健康支援中心为改善咨询活动和提高支援效果，进行各种调查研究活动。因此，可能会使用匿名化后的信息在学会或学术期刊上发表研究成果。当信息被用于个别研究时，将会在校园生活与健康支援中心的官网上通知大家，请随时查阅。
